___ Three copies of each entry that don’t have the author’s name printed anywhere on them.

___One cover sheet that includes

            ___Author’s name

            ___Address

            ___Phone

            ___Email address (double-checked for accuracy)

            ___Title and category of submission or submissions..

            ___Brief biography (one paragraph)

___Appropriate fees ($15 per submission non-TWA, $10 per submission TWA member) Make check out to Tallahassee Writers Association.

___SASE (ONLY if you don’t have an email address that you plan to keep until the end of the year and you want to receive a list of winners.)
